Main – Pediatric Orthopedics: Legg-Calvé-Perthes disease and the Use of External Fixators in Orthopedics and Traumatology

Summary

Pediatric Orthopedics has made enormous progress. Advances in the areas of molecular biology and genetics have led to a better understanding of childhood orthopedic disorders. New technology with implant materials has spurred the surgical treatment of fractures and correction of limb deformities. The development of preventative measures in childhood traumatology, in the screening for developmental dysplasia of the hip (DDH) and in early treatment of disorders such as congenital clubfoot prevention must evolve.

The following areas are part of clinical and experimental research in the discipline and are in research development and planning:

Legg-Calvé-Perthes disease – comprehensive thematic project, with international participation in the IPSG – International Perthes Study Group, based in Dallas, Texas, USA, at the Texas Scottish Rite Hospital. Collaborative studies with the IEPE – Teaching and Research Institute of the Hospital Sirio Libanês, São Paulo, with the creation of an experimental animal model of the disease. Clinical studies with new diagnostic resources in an attempt to clarify the etiology of the condition. Systematic review of the surgical treatment outcomes.

Developmental Dysplasia of the HIP (DDH) – There are numerous aspects of the condition that are still controversial and require a sequence of investigations. For example: screening or not screening for the condition; how to avoid the costs and complications arising from a late diagnosis of the disease; how to improve surgical treatment.

Osteogenesis imperfecta: the basic pathophysiological alteration that patients present is osteopenia/osteoporosis. In order to improve their bone mass, experimental and human studies on the possibility of using drugs that improve the quality of bone tissue are and will continue to be a research goal. For orthopedic surgery of patients with OI, we are studying and researching corrective osteotomies of the long bones using intraspinal devices, with and without elongation mechanisms.
